APPOINTMENTS
Patients are seen only by appointment. Before your first visit with me, please complete all of the First Visit Forms and bring them to your appointment.
Initial visits will usually last about 40 minutes. Medication management visits may last about 15 minutes. Counseling visits usually last 45 minutes. The type of follow-up appointment indicated for you may be discussed during your initial evaluation.
I am not obligated to wait and keep your appointment if you are more than 15 minutes late to an appointment. If you are late, the appointment might not extend past the allotted time.
CANCELLATIONS
When you schedule an appointment, that time is reserved specifically for you. It is your responsibility to remember and keep scheduled appointments. A minimum of 24 hours notice is required if you are canceling or rescheduling an appointment. Missed appointments and appointments which are canceled with less than 24 hours notice will be billed to you as an out of pocket charge (health insurance will not pay it).
EMERGENCIES
An emergency is a serious situation requiring immediate attention. If you experience what you believe is a psychiatric emergency and would like to talk to me, you may reach me by calling 808-386-1866 24 hours a day, 7 days a week. Other options include calling 911, going to the nearest emergency room, or calling the 24 hour ACCESS line at 832-3100. ACCESS staff can provide suicide and crisis intervention as well as link you to appropriate services.
REFILLS
If you need a refill, please email me at [email protected] or contact your pharmacy; your pharmacy can get in touch with me. If I have not evaluated you recently, I may need to see you before I can refill your medication.
